The Illinois School Code has specific immunization, physical, and dental examinations that are required to attend school that must be provided at the following times: Pre-K, Kindergarten, 2nd grade, 6th grade, 9th grade, 12th grade and upon first entry to an Illinois school.

Health requirements are due the first day of student attendance (August 17, 2022). Incomplete health records will result in exclusion from school.
Health documentation may be dropped off at your student's school during regular business hours beginning on August 1st or emailed to their respective school nurses (see below).
